The purpose of this form is to submit your high-impact professional achievements or educational milestones that go above and beyond your job responsibilities. These achievements may be completed either internally or externally. Examples of high impact achievements are completion of a certificate/degree, becoming a chair/board member of a professional association, presenting at a professional conference and/or receiving a grant. - The Staff Retention Advancement and Training (SRAT) subcommittee of the President’s DEI Committee has discretion to review the appropriateness of submissions for this form.
